CRC Health Centre in Richmond says it takes a collaborative approach by bringing medical, pharmacy and home-care services together under one roof.
The centre describes its model as team-based, with family physicians, specialists, pharmacists, nurses and other healthcare professionals working together to share knowledge and coordinate care for patients with different or more complex needs.
CRC says it offers opportunities for mentorship and discussion, pairing experienced practitioners with newer members of the healthcare community to support work involving complex patient needs.
It says that for some patients, care continues after a medical appointment through pharmacy prescription and medication-related services and through home-care support for patients who need assistance at home, including seniors and people with more complex care needs.
The centre also states that its home-care program was established to provide additional support for patients who need ongoing assistance at home.
CRC Health Centre says its clinic environment includes administrative and operational resources intended to give doctors more opportunity to focus on patient care, including for scheduling, billing and other administrative responsibilities.
The centre says it provides care in English, Mandarin and Cantonese.
